I'm not sure that my code was the problem. I think that editing the file in Dreamweaver caused the file to be uploaded incorrectly. I am testing DW2004 and double clicked on the .htaccess file on my hosted website. Rather than opening in 2004, it opened DW MX [the file had a DW file type icon on my local computer, but a blank icon online]. DW MX opened the file and I edited and uploaded the file. Then none of the custom .htaccess worked, even when I deleted the newly added code and returned it to its originally working state.
I then opened the file via remote ftp using BBedit and saved it, and added the code and all appears to be working correctly.
I suspect that somewhere DW confused the file type upon upload.
